As a current / vested participant of the Beer Industry-Local Union No. 703 Individual Account Plan (IAP), you are entitled to request an Annuity Statement, as of the last Valution Date, that shows the following:

IAP APPLICATION
If you are vested and no longer receiving contributions into your Individual Account Plan, you may apply to receive this benefit in the form of a one-time lump sum payment via check or roll it over into an IRA of your choosing.
Please complete the below application and return to the Fund Office along with a copy of your photo ID. If you are choosing a Direct Rollover, you MUST furnish us with documentation from that financial institution, verifying the account and authorizing the rollover.

QUALIFIED DOMESTIC RELATIONS ORDER (QDRO)
It is recommended that the below QDRO Sample and Procedures be followed by you and your attorney. You may have a draft submitted to the Fund Office for review prior to its entry in court
